Summary:
Responsible for working as part of a multidisciplinary team on the inpatient medical, ICU and ED units executing elements of the discharge plan, arranging and facilitating team/patient/family meetings and providing resources while under the supervision of licensed Care Management staff. Develops awareness of community resources to meet the needs of patients based on treatment factors and population health.

Other Information:

  • Both prior experience working in a hospital setting and bilingual skills is a plus. 
  • Experience using electronic medical records is necessary, administrative experience desired.
  • Weekend rotation duties
  • Demonstrated experience working in multicultural and multidisciplinary settings required.
  • Cognitive ability: Ability to multitask and exceptional organizational skills required
