CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
As a member of the PACE Interdisciplinary team, the Activities Coordinator II develops and implements a therapeutic activities program designed to meet the individual physical, social, emotional, spiritual, and intellectual needs of participants in a PACE adult day health setting.
Job Duties and Responsibilities: Contributes to the creation of care plans for participants and new enrollees by performing individual leisure assessments and participant outreach on a scheduled basis at the ESP Care Sites or in the community. Interacts with participant's caregiver/family members as needed for collateral contact, orientation to the day center, scheduling, etc. Researches, plans, and carries out activity programming that meets the individual physical, social, emotional, and intellectual needs of participants. Completes activity schedules and supply ordering by established deadlines. Leads a variety of groups based on the social, emotional, spiritual, physical, and cognitive well-being of the participants. Orients new participants to activity program. Assists with juice/coffee services, lunch, transportation if needed. Demonstrates ability for creative arts activities. Effectively manages behavioral episodes/conflict among participants. Responsible for the completion and maintenance of appropriate records and documentation. Responsible for contributing to, reading and following participant care plans.
High school diploma; BA in related field desirable
Nationally recognized certification as an activities professional such as Activity Assistant, Activity Director, Recreation Therapist highly desirable
At least one-year experience working with frail elders in a social/medical setting Experience developing and running therapeutic activities preferred
Fluency in Spanish, Portuguese and/or Haitian Creole highly desirable
