Department Description:
CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
Summary:
This is a Homecare Coordinator role on our PACE team. The job-specific duties and responsibilities are as follows:
- Schedules all home care referrals to meet the needs of PACE participants.
- Initiates homecare referrals for pre-authorized home care.
- Works with Homecare Manager, IDT and Case Workers to prioritize home care resources.
- Effectively communicates with participants and their family/caregiver (as appropriate).
- Records, maintains, monitors and verifies accurate home care records, including service documentation, medical records, billing pre-approvals, etc.
- Provides support to participants through empathic, trusting relationships.
- Demonstrates respect for differences in language, culture, race, religion, citizenship, gender, and sexual orientation and does not discriminate on the basis of income, insurance status, immigration status or disability.
Minimum Qualifications:
Education: High school/GED required; Associates Degree preferred
Work Experience: Minimum 3-year Administrative experience in a medical office setting.
Fluency in Microsoft applications such as advanced word processing, spreadsheets, databases, flowcharts and graphics (Microsoft Office and Google apps), writing, interpersonal, organizational and communication skills required.
