The Case Worker for the Outpatient Psychiatry Department (OPD will help individual clinicians, and the overall department, by working directly with patients to assess and identify all psychosocial needs, help them and their families access vital services to address these needs, and consolidate available resources to facilitate these connections. By providing this support , the OPD Case Worker should help improve both patients' and providers' experience of care, enable Mental Health clinicians to spend more time delivering professional services, and connect patients to services that are vital for improving their overall health, functional status, and quality of life.
To this end the Case Worker will:-
Reliably connect patients and families to community health resources and services
Help patients apply for/obtain clinical support services (e.g. DMH, health insurance, referral to other levels of care)
Help patients apply for/obtain social support services (e.g. housing, legal, transportation)
Help develop and maintain an inventory of CHA and community resources that can support patient care
Coordinate case management efforts with other case worker/managers in Psychiatry
In conjunction with Population Health, will support the CHA outpatient psychiatry department in diverse efforts to address top strategic priorities for improving the overall health of our patient population, addressing gaps in care for individual patients, and improving performance on key process and outcome measures to help CHA achieve care transformation incentives through contracts with MassHealth. The Case Worker - Population Health will work closely with a wide range of Psychiatry staff in clinical and non-clinical roles, including operational leaders, quality managers, administrative staff, and other clinical support providers (e.g. case managers, care partners, medical assistants).
Individuals with serious mental illness (SMI) and/or substance use disorder (SUD) often receive various levels and types of services from multiple service providers, which can complicate their care and drive poor outcomes. One critical feature of the Case Worker - Population Health position is managing and improving processes to coordinate information exchange, care planning and care transitions with providers beyond outpatient psychiatry, including other CHA providers (e.g. primary care, inpatient units, emergency departments, intermediate care) and external providers of social and clinical services.
Education/Training: Minimum of a Bachelor's degree in social work, psychology, public health, counseling, health education, or similar fields.
Work Experience: Minimum 2 years in health care setting collaborating with clinicians and administrators; experience in behavioral health settings and direct interaction with service users, preferred.
Must speak fluent Spanish
