  • The Elder Service Plan of the Cambridge Health Alliance is a Program of All Inclusive Care for the Elderly (PACE). ESP of CHA serves adults 55 years old and older with chronic medical and mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. ESP of CHA's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services.

    Job Purpose: Participate in delivering and arranging for culturally, linguistically, age, etc. appropriate pastoral care to patients/residents and their loved ones at a PACE site, the participant's home, hospital, nursing home or to particular faith groups as assigned. Actively participate in the delivery of pastoral care, with focus on end of life support. Completes spiritual assessments. Respond to requests for pastoral care in a timely manner. Participate in the offering of worship opportunities. Participate in the meeting ritual and sacramental needs. May develop and lead patient spiritual groups based on needs of PACE participants Performs some on-call chaplaincy services. Support an interdisciplinary approach to pastoral care. Participate in team, family, etc. meetings. Enhance staff sensitivity to the spiritual needs of participants, families, etc. Serve on end of life and other committees as requested. Promote an interdisciplinary approach to care Participate in maintenance of proper administrative procedures in accordance with CHA policy. Assist in the coordination of clergy and pastoral visitors from the religious community. Record notes in electronic medical record to communicate with CHA, etc. Participate in the department's plan for continuous quality improvement. Performs other duties as assigned or requested.

    Requirements:

    MA of Divinity, MA Arts in Religion, or MA of Pastoral Counseling Board Certified Chaplain (BCC)

    Certified by and in good standing with the Association of Professional Chaplains, the National Association of Catholic Chaplains, or the National Association of Jewish Chaplains
