  • The Elder Service Plan of the Cambridge Health Alliance is a Program of All Inclusive Care for the Elderly (PACE). ESP of CHA serves adults 55 years old and older with chronic medical and mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. ESP of CHA's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services.

    The Social Worker is a member of the Elder Service Plan Interdisciplinary Team who provides clinical social work services to participants at the ESP Center and in the community. The Social Worker, along with other members of the Interdisciplinary Team, work to carry out the participant's individual comprehensive care plan.

    Clinical Social Worker will provide in-home and clinic based clinical case management, assessment, crisis intervention, supportive counseling, and community resource coordination for frail elders as part of an interdisciplinary team working in the PACE model of care. PACE provides all-inclusive medical and social care to frail elders in an attempt to help them remain as independent as possible in the community. Attend daily morning meetings. Complete initial, semi-annual, and annual assessments. Provide supportive visits. Follow up with family and collaterals including participant's housing. Triage urgent issues.

    Current Massachusetts social work license in good standing
    At least 1 year experience working with elders.
    Experience in medical social work, elder community services, behavioral health, case management, etc. preferred.
