  • CHA is seeking an LICSW to join our Primary Care Mental Health Integration team. This is a terrific opportunity to participate in a system-wide transformation program offering behavioral health services in an interdisciplinary team-based setting. Team members include primary care providers, physician assistants, nurses, complex care managers, care partners, psychiatrists, psychologists, and social workers.

    Responsibilities: Therapists provide time-limited psychotherapy and potential for groups in a fully integrated primary care setting, utilizing a solution-focused approach, CBT interventions/protocols, behavioral activation and motivational interviewing techniques as indicated. Therapists provide brief treatment and support primary care conducting risk assessments and determining the appropriate level of care. They also provide evaluations and referral to other levels and steps of care. Therapists participate in staff meetings and seminars alongside their teams and have role in consult/liaison in the CHA system. Must maintain timely documentation within our EMR (EPIC), including treatment plans.


    Qualifications: LICSW with experience in providing cognitive behavioral treatment on outpatient settings, and familiar with the public mental health delivery system required. Experience providing consultation to medical providers, and in working with multidisciplinary teams.
