Temporary Clinical social workers are vital members of the interdisciplinary care teams in ambulatory health settings. In this position the social worker is responsible for the following: * Serve as the first line social worker to address ad hoc needs in both inpatient and ambulatory settings based on the Cambridge, Somerville and Everett communities. In addition to the inpatient units these settings include but are not limited to: Cambridge Pediatrics, Orthopedics, Medical Specialties, Surgical Specialties, Cambridge Women's Health, Surgical Day Care, Windsor, Union Sq, East Cambridge, Cambridge Family Health, Somerville Hospital primary Care, Malden Health Center, Revere Health Center. * Provide focused assessment and brief intervention for patients and families based on reason for referral. * Provide patient centered care coordination services within CHA and community based agencies to support patient/family needs. * Provide risk assessment as required on cases concerning substance use disorders, mental health screening and referral, child/elder abuse and neglect and domestic violence for referred patients. Qualifications/Experience Education/Training: Masters degree in Social Work or an equivalent degree in a related field. Licensure: Current MA licensure as LICSW (Licensed Independent Clinical Social Worker). Certifications: Same as above. When required, must be credentialed by the CHA Medical Staff for practice in relevant clinical practice areas, such as individual, couple, family, or group psychotherapy. Work Experience: Minimum of two years post masters degree, in clinical care setting with relevant populations.[cha111813]
