Job Purpose: Clinical duties will be provided at medical sites, including assessment. evaluation, treatment and or referral of children and adolescents with mental illness. The primary treatment modalities will include consultation, short term, group and intermittent therapies and triage to other services as possible for patients who need long term treatment. Provider will participate in chronic disease management programs such as diabetes, depression and other chronic conditions. Works with specialists and generalists who encompass multiple skills, cultural competencies and therapies with various modes of interventions with basic and/or advanced skills in multiple therapies. May work on specialty staff team with focused subspecialty work group to teach, perform research and learn new approaches including: CBT, DBT program, Trauma, Addictions, Cultural/Linguistic, Family Therapy Offers treatment recommendations for specific diagnosis while educating the child or adolescent and parents about illness and treatment options. Contributes as a member of team in discussions of diagnosis and treatment. Shares problems and considers issues of acuity and risk. Participates in the assignment of mental health and addictions care options based on clinical practice criteria. Shares and participates in the knowledge acquisition process of new initiatives, policies and practice. Maintains awareness of medical issues and/or problems of patients to ensure patients have preventative and secondary care. Collaborates on cases through the sharing of assessments and framework of treatment empowering PCP group to improve interventions. Collaborates with multi- and timely fashion disciplinary team at service site. Provides all necessary documentation in clients' medical charts in an effective. Participates in teaching and training activities including continuing education activities.
Education/Training: Master's degree,
Social Work Licensure: Current MA licensure as LICSW (Licensed Independent Clinical Social Worker). LCSW (Licensed Certified Social Worker) considered with eligibility for licensure of LICSW within one year.
Certifications: Same as above. When required, must be credentialed by the CHA Medical Staff for practice in relevant clinical practice areas.
Physical Skills: Ability to negotiate the physical space of the clinical setting and to utilize computerized documentation.
Mental Skills: Analytical, organization, observational and documentation skills. Integration of theory and practice.[cha101716]
