Under the guidance of the Senior Director and in collaboration the ACO Program Development Manager, the Flexible Services Program Coordinator provides support for program operations of the MassHealth Flexible Services Program at CHA.
This position is a two-year, DSRIP-funded position.
1.Coordinates operations and monitoring of the MassHealth Flexible Services program. 2.Coordinates program referrals and patient navigation to services as a liaison between CHA clinical departments and Flexible Services vendors. 3.Documents referrals and progress using electronic tools, including RedCap, Epic and Excel. 4.Delivers select Flexible Services that are based at CHA. 5.Navigates patients to Flexible Services as liaison between CHA and nutrition and housing services vendors. 6.Ensures proper documentation and detailed procedures are carried out as defined in Flexible Services program operations. 7.Verifies service delivery and reconciles vendor reports and invoices. 8.Participates in performance improvement activities as necessary. 9.Coordinates the work of identified departments and team members in implementing change. 10.Facilitates monthly meetings with Flexible Services vendors. 11.Develops and collaborates with Flexible Services Work Group. 12.Prepares necessary internal and external reports, including written narratives of accomplishments and statistical analysis of results. 13.Provides input into performance evaluations of Flexible Services vendors. 14.Provides support for other projects and initiatives as directed.
Education/Training Graduate of an accredited college/university with a BS/BA or equivalent degree required; Masters degree in health care, clinical discipline or management related field desirable.
Certification/Licensure PMP desirable.
Preferred Work Experience Three years of progressive experience working in a health care, payer or policy setting. Project management experience desirable. Experience in client relationship management required. Demonstrated excellent problem solving, interpersonal, communication, team leadership, priority setting, organizational and work competency skills. Ability to present information in an articulate, organized and professional manner. Experience with information systems and software packages typical to a health care setting and project/business analytics environment (e.g. Microsoft Office products, electronic medical records, database management, etc.).
