  • The Elder Service Plan of the Cambridge Health Alliance is a Program of All Inclusive Care for the Elderly (PACE). ESP of CHA serves adults 55 years old and older with chronic medical and mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. ESP of CHA's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services. The PACE- Interdisciplinary Team Supervisor will be responsible for one interdisciplinary team and supervising/supporting one clinical discipline.

    Job Specific Duties/Responsibilities: Demonstrate proficiency in PACE/managed care model and PACE regulations. Facilitates IDT discussions (morning meeting and care plan) and oversee IDT activities and delivery of care. Coordinate work schedules, approve payroll, and follow-up on day to day tasks of supervised staff, including clinical competencies of discipline specific staff. Complete performance reviews and any necessary counseling (discipline) in collaboration with discipline specific supervisor. Initiate and coordinate hiring of IDT staff in collaboration with discipline specific supervisory staff and leadership team. In conjunction with IDT, work to achieve participant care plan and wellness goals, as well as program utilization goals such as minimizing excess use of hospital, short-term rehab, and long term care services. Oversee discharge planning and care coordination activities across all levels of care. Educate and supervise IDT in managing complex cases involving multiple areas of psychosocial need. Maintain relationships with outside quality and utilization stakeholders, such a wellness nurses in assisted living communities, DONs, etc. at nursing homes, etc. by initiating and attending quarterly and/or monthly (as appropriate) meetings with community partners. Participates on ESP quality assurance / performance improvement, utilization review, operations, enrollment and other committees as assigned. In conjunction with Quality Manager and committee, collaborates to achieve goals outlined in annual Quality Assurance Plan. Take administrative on-call phone according to on-call schedule.


    MSW, OT, PT, RD, RT, SLP or equivalent PACE (Program for the all-inclusive care of the elderly) interdisciplinary team clinical training

    3-5 years of supervisory experience, preferably in a managed care, PACE or equivalent healthcare setting with responsibility for achieving quality or utilization benchmarks

    Training or advanced degree in health administration desirable
