  • The Elder Service Plan of the Cambridge Health Alliance is a Program of All Inclusive Care for the Elderly (PACE). ESP of CHA serves adults 55 years old and older with chronic medical and mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. ESP of CHA's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services.

    The PACE Psychotherapist is a supporting member of the Elder Service Plan (ESP) Interdisciplinary Team who carries a psychotherapy caseload of ESP participants, provides clinical supervision for the social work department, and oversees the behavioral health needs of participants at the ESP Center and in the community


    LICSW will provide clinic-based individual and group psychotherapy and crisis intervention for frail elders as part of the PACE model of care and provide individual and group clinical supervision. PACE provides all-inclusive medical and social care to frail elders in an attempt to help them remain as independent as possible in the community. Collaborate with interdisciplinary team and attend team meetings as needed. Triage urgent issues.


    LICSW required

    At least 5 years clinical experience with at least 1 year experience working with the geriatric population.

    Experience in medical social work, elder community services, behavioral health, case management, etc. preferred.
