1. a - Provides assessment of patient educational needs and education in all aspects of diabetes and self management for the patient with diabetes in both individual and group settings and includes families as indicated. Education takes place in the ambulatory setting , providing information on the following using the National Standard for Diabetes Self Management Education as the guideline. Diabetes disease process: Type 1 and Type 2 Diabetes in adults across the age spectrum (18 years and older) Complications of diabetes o acute ( glucose extremes). DKA Hyperosmolar state chronic / long term risks Medication management; oral medications, insulin, non insulin injectable medications o Insulin injection teaching o Insulin pump Role of food management Role exercise and activity Psycho-social issues impacting diabetes including Self monitoring; Using the results, pattern management o glucose meters o CGM devices - long term & diagnostic Goals for glucose, weight, blood pressure and cholesterol and other as indicated Setting personalized goals and planning to achieve goals. Diabetes treatment options including medications, lifestyle changes, monitoring options and insulin delivery options. Patient data management options Understanding the health care system 1. b - Documentation: All education encounters are documented in the patient medical record indicating content taught, patient response, goals identified and set, met or not met. Documentation also includes review of current medications, vital signs as indicated and follow up plans. 1.c - Provides appropriate education materials based on patient preference, language, literacy and cultural background 2. Able to individualize instruction based on the needs of the patient based on identified barriers, cultural influences and lifestyle. 3. Timely and regular follow up of the patient to support patient meeting goals over time. 4. Communicates with the referring provider
Requirements: A graduate of an accredited school of nursing. Newly hired RNs are required to have a BSN within 1 year of hire. BSN Preferred. MASS RN Licensure required. AHA BLS required.
Experience: Minimum of 5 years of nursing experience including providing patient education in the ambulatory and inpatient setting. Certified Diabetes Educator (CDE) required.
