Summary:
CHA’s Department of Community Health Programs are grant and non-grant funded programs that increase linkages between patients, communities and health care, social determinants of health, outreach, training, and health education on topics such as navigating the US healthcare system, aging and emotional wellness, and communicable and non-communicable diseases or conditions such as hypertension, cancers, or behavioral health. Periodically, DCH will need temporary support to meet grant requirements based grant deliverables.
Responsibilities:
Coordinate and conduct focus groups, in Spanish/English, on patient motivation and compliance with health directives
Develop, coordinate, and implement culturally and linguistically tailored outreach and educational programs and activities including wellbeing classes and workshops.
Coordinate program logistics and administrative support to ensure smooth execution of programs and educational sessions.
Develop protocols and guidelines for program implementation to support programs.
Develop strategies to enhance cultural responsiveness, accessibility, and inclusivity of programs.
Serve as faculty for CHW or MHFA training related to program work and group facilitation.
Create educational materials, flyers, and posters to promote programs and activities.
Work with the marketing team to promote upcoming programs and provide post-event photos and summaries.
Maintain regular communication with internal teams and external partners to align program goals and logistics.
Develop protocols for and coordinate use of social media groups such as through WhatsApp, and train DCH team leads in the use of the social media
Collect and manage program data, including participant attendance, surveys and engagement metrics.
Support the Program Director or Evaluation Specialist with grant and reports
Ensure program activities comply with applicable laws, regulations, and organizational policies, including privacy, confidentiality, and safety standards.
- Review program materials and procedures to maintain compliance with local, state, and federal regulations.
MINIMUM QUALIFICATIONS :
Other information:
Bachelor’s degree or advanced medical training required (US or foreign)
Completion of CHW training.
Previous experience working with the community (volunteer, training or paid work allowed.)
Demonstrated experience working with programs that serve diverse communities.
Experience working with Community Health Workers or in community-based health programs.
Written and oral skills in English
Strong knowledge on the google suites
Ability to work collaboratively in a team and independently manage multiple projects.
Excellent communication, organizational, and problem-solving skills.
Competency with data collection and analytics tools, excellent computer skills (word processing, internet research, presentation and reports )
Written and spoken proficiency in at least one other language (preferably Spanish)
