Department Description
The Cambridge Public Health Department (CPHD), a recipient of the 2013 Robert Wood Johnson Foundation Culture of Health Prize, works to protect and promote the health of everyone in the city, with a special focus on vulnerable populations. Administered by the Cambridge Health Alliance, the CPHD uses a Social Determinants of Health framework to address factors like economic stability, education, and racism that affect community well-being. Its key areas of focus include communicable disease prevention, environmental health, and emergency preparedness. The department is also responsible for creating and enforcing public health regulations within Cambridge.
The School Health Nursing program, a collaboration between the Cambridge Public Health Department (CPHD) and Cambridge Public Schools, supports students in grades K-12 by promoting health and academic success. Staffed by registered nurses, vision and hearing specialists, health assistants, and dental program staff, they provide first aid, medication administration, health counseling, and education, while also ensuring state mandates are met. The program develops care plans for students with chronic illnesses, connects families to healthcare resources, advises school staff, and verifies immunizations. Full-time coverage is provided in all public schools, with healthcare oversight and clinical guidance extended to three nonpublic schools, city-managed preschools, and camps.
Job Summary:
Nurse Case Manager will address the needs of students with chronic health conditions so students with these conditions may attend school safely.
Nurse Case Manager will assist in initiating a “Bridge” re-entry program, with adjustment counselors and other school staff, for students returning to academics after extended absence for reasons such as post hospitalization, rehabilitation or recovery, or concussion, to successfully return to learning.
Nursing Case Manager will assist in implementing additional projects such as responding to state and local emergencies, pandemic response, changes in state mandates and requirements for school health services, etc.
Nurse Case Manager provides an annual community health needs assessment that addresses health inequities and the impact of institutional racism and socioeconomic disparities on students and their academic achievement.
Duties the Nursing Supervisor deems necessary and assists in any and all aspects of grant requirements.
Requirements:
Registered Nurse in good standing with the Board of Registration in Nursing
Current MA RN Licensure required
Bachelor of Science in Nursing (BSN) degree required
Current Basic Life Support for Healthcare Providers by the American Heart Association or American Red Cross is required.
Pediatric experience and/or Emergency/Public Health experience. Experience in Case Management preferred
