At CHA PACE, our mission is to support our program participants in achieving the highest level of independence within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment.

Position Summary

The PACE Program Manager – Interdisciplinary Team (IDT) is a key operational and clinical leader responsible for the effective management and performance of an assigned interdisciplinary team within CHA PACE. This role provides leadership and oversight of interdisciplinary care coordination, team operations, quality improvement, and regulatory compliance. The Program Manager facilitates effective team collaboration, addresses complex participant care and staffing concerns, and promotes accountability and continuous improvement.

Working closely with the Program Director of Quality and Safety and other PACE leaders, the Program Manager supports quality and compliance initiatives through data collection and analysis, incident investigations, root cause analysis, audit preparation, and implementation of improvement strategies.

The ideal candidate is an experienced healthcare professional and collaborative leader who thrives in a complex, interdisciplinary environment and is passionate about improving the health, well-being, and independence of older adults.
