Summary: 

Responsible for working as part of a multidisciplinary team on the inpatient medical, ICU and ED units executing elements of the discharge plan, arranging and facilitating team/patient/family meetings and providing resources while under the supervision of licensed Care Management staff. Develops awareness of community resources to meet the needs of patients based on treatment factors and population health. 

Requirements:

  • Both prior experience working in a hospital setting and bilingual skills is a plus. Experience using electronic medical records is necessary.
  • Education/Training: College degree or equivalent preferred. Relevant experience may be considered in lieu of degree human service field. Although licensure is not required, training in social work to include a BSW degree and/or a LSW license is desirable. Weekend rotation duties.
  • Work Experience: Experience working with relevant community populations, such as, people with chronic illness, cognitive disorders, substance abuse. 
  • Other as assigned.
