Department Description: 

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Job Purpose: 

The LPN Home Care Nurse is a member of the PACE Interdisciplinary Team (IDT) who provides nursing care to participants in their homes who have a wide variety of diagnoses. Nursing care is reflective of the participant's plan of care. Participant's status, new problems, and/or problems indicating a significant change in status are communicated to the RN.  This is a home care/clinic rotation role.

Qualifications:

  • Graduation from an accredited licensed practical school of nursing
  • 1-year experience in geriatrics preferred
  • 1- 2 years home care experience preferred
  • Maintains current Basic Life Support (BLS) certification 
  • Knowledge of a second language preferred
  • Must have a car to travel to participants’ homes
